State regulation, 4 AAC 19.015, requires districts to make their certificated employee evaluation template(s) available to the public. After reviewing evaluation tools used by other school districts, Cordova School District determined the documents below best meet the necessary requirements for acquiring pertinent information regarding staff performance.

Public Complaint Form

Occasionally, the public has an individual concern about a program, practice, or procedure within the District. This concern may have arisen out of a contact from a school patron or may be a matter that the Board member simply wishes to be reviewed by the administration.

Individuals are encouraged to use this form freely, but keeping in mind the time demands it places on school administrators. All concern forms are to be processed through the Superintendent who may delegate other administrators to follow up on the matter within fifteen (15) working days.
